Some health insurance carriers
like Medicare require extensive paperwork from your physicians. We can help with this and all
your medical equipment paperwork. Call Majors Mobility at 1-800-570-3393 for more information.
NOTE TO MEDICARE RECIPIENTS: If you have Medicare, your scooter may be covered if you need a scooter to perform
activities of daily living (such as cooking,
cleaning, bathing, dressing, toileting etc..) within your residence because of a medical problem that limits your
ability to walk and you are unable to self-propel in a manual wheelchair. Your physician will be required to
document the need for a scooter in order to submit your claim for payment. We can help with this and all the
other paperwork to make it as easy on you as possible. The typical process for obtaining a scooter
through Medicare goes like this: |

Schedule an appointment; then visit with your doctor to talk over your need for a scooter (why you
need it in your home and why you can’t use a manual wheelchair). If the doctor agrees and can
document your need for the equipment, have him/her write an initial order (RX).

Call Majors Mobility
for an in-home assessment and evaluation. Majors will request copies of your doctor’s chart notes
that support the need for the scooter.

Majors will visit you in your home to help determine the
best equipment for you, your environment, your goals, and your lifestyle.

Majors will send/fax
a detailed equipment list to your doctor to sign.

Once the paperwork is returned completed and
you qualify, we will order your scooter or set up delivery from a scooter in our stock.
